if an icon is not defined in the css file it results in rectangle, the css file you imported might depend on the other assets as well and you are importing only css file, try cdn or download all the necessary assets not only css file itself

The problem I'm facing is that contenttype is not being created for one of the databases A - contenttype exists B - missing

here you do not have action attribute... like which url handles request when user sends form back to back-end side...

only integers can be auto-incremented, you have to do it manually, but in fact what is the need of emp001, emp002, just 1, 2 suffice as long as it doesnt mean smth important
